Waiting period - 3 months of continuous employment
Termination - Termination of coverage when you retire or reach age 75

Drug substitution limit:
Charges in excess of the lowest priced equivalent drug are not covered unless specifically approved by Sun Life. To assess the medical necessity of a higher priced drug, Sun Life will require the covered person and the attending doctor to complete and submit an exception form.
In-province hospital
- 100% of the difference between the cost of a ward and a semi-private hospital room
Convalescent Hospital
- 100% of the difference between the cost of a ward and a semi-private room, up to $20 per day for a maximum of 180 days for treatment of anillness due to the same or related causes.
Out-of-province emergency services

Paramedical services

- licensed osteopaths or osteopathic practitioners, including a maximum of one x-ray examination each benefit year
- licensed podiatrists or chiropodists, including a maximum of one x-ray examination each benefit year

- licensed psychologists or social workers or licensed psychotherapists, or psychotherapists who are active members of a provincial association approved by Sun Life

Maximum Benefit
- $750 per person benefit year maximum
- If you apply for coverage either for yourself or your dependents more than 31 days after becoming eligible, the maximum benefit is $100 per person during the first year for all
expenses.
Long-Term Disability:
Maximum amount:
66.67% of your monthly basic earnings up to a maximum of $5,000. For coverage in excess of the amount indicated under Proof of good health, your coverage is subject to approval by Sun Life. Refer to Proof of good health below for further information.
The maximum amount may be reduced by benefits and payments provided from other sources as described in the Long-Term Disability section of your booklet.
Tax Status:
Your employer has indicated that all employees covered under this disability plan are paying the full cost of the premium. Therefore, the benefit payments are not taxable income.
Proof of good health:
Approval required for coverage in excess of $2,750, and any increase in that coverage of 25% or more or $500, whichever is greater.
Elimination period:

Maximum Benefit Period:
The period ending on the last day of the month in which you reach age 65.
Benefits may also end on an earlier date as specified in the Long-Term Disability section of your booklet.

To access Sun Life's online doctor benefit, known as Lumino Health Virtual Care, follow these steps:
Visit the Website or Download the App
Go to luminovc.dialogue.co or download the Lumino Health Virtual Care app from the App Store or Google Play.
Register Your Account
Sign up using the insured person's name, birthdate, and the postal code from your welcome letter. This will grant you access to the platform.
Once registered, you can use the platform to:
- Consult with doctors, nurse practitioners, and mental health professionals.
- Receive prescriptions.
- Get referrals to specialists.
- Access mental health support through the Stress Management and Well-Being program.

Vacation & sick leave policy

Public/Statutory Holidays
In accordance with the Ontario Employment Standards Act,
C2inc. observes the following paid public holidays:

Employees are eligible for holiday pay if they are present on the workday before and after the holiday. Exceptions include absences due to illness (supported by a doctor’s note) or if an employee is on vacation. If a holiday falls on a weekend, management will follow provincial or federal guidelines to offer an alternative day off.
Vacation:
We believe taking time away from work is essential for overall well-
being.
Employees are encouraged to use their vacation days for relaxation,
personal time, family obligations, or any other personal matters
requiring time away from work.
Vacation Year: The vacation year runs from January 1 to December
31. Vacation days must be taken within the year they are earned.
Vacation Entitlement: Vacation days are based on years of service,
as follows:
Public Holiday During Vacation: If a public holiday falls during your vacation period, you will be given an additional day off.
Requesting Vacation: Employees must submit vacation requests through the Collage HRIS system for management approval.
Management may also schedule vacations to ensure that all employees take their vacation time before the year ends, with at least two weeks' notice provided.
Vacation Carryover: Employees must use their vacation days within the calendar year. Unused vacation days cannot be carried over to the next year.
Termination: If employment ends before the year is completed, any vacation taken beyond the prorated entitlement will be deducted from the final pay.
Sick Leave
“At C2inc., we provide paid sick leave to support employees when they are unable to work due to illness or medical needs.”
Sick Leave Entitlement: Employees are eligible for up to five paid sick days per calendar year. Sick days may be taken in half-day or full-day increments and cannot be carried over from year to year.
Requesting Sick Leave: Employees must notify management at least one hour before the start of their shift if they are unable to attend work due to illness. For medical appointments, at least one week’s notice is required.
Doctor’s Note: A doctor’s note may be requested in cases where an employee exceeds their sick leave entitlement or if there is a pattern of frequent absences.

Wedding Bonus
This vacation bonus is intended to support wedding planning, the celebration itself, or your honeymoon.
